Custom Forms

[Enhancement] Custom Forms: We have made some exciting new enhancements to our Custom Forms tool based on client feedback. These changes add tighter links between creating, sending, and reviewing form responses, give users more information about forms at a glance, add a brand new locked forms feature, add a new “Select All” button to the form builder, and enhance the overall user experience.
The enhancements to the Manage Custom Forms page provide users with more information about forms at a glance, and allow users to jump directly to completing additional actions on a form.

Complete list of changes:
- Moved “Create New Form” button from the sub-nav bar to the page.
- Added a “Created By” column, which shows the name of the user who created the form.
- Added a “Lock Status” column. For more information on this feature, see below.
- Added a "Recipients" column, which shows the number of forms sent out, and which status those forms are in.
- Grouped actions into a single actions menu.

The Create/Edit Custom Form page remains mostly unchanged, but plays host to our new “locked forms” feature! The new locked form feature allows users who created a form to lock that form from being edited by other users.

- Moved navigation buttons out of sub-nav bar and onto the page when editing a form.
- Re-labeled the “Save” button to “Create and Continue” when creating a new form.
Locked Form Feature
The new Locked Form feature allows a form creator to “lock” that form from being edited by other users. When creating a form, the form is unlocked by default. To lock a form, the user can navigate to the create/edit form page for a form, and set the “Is Locked?” field to “Yes”.
If a form is locked, users other than the form creator cannot edit, delete or unlock the form. However, users other than the form creator can still preview, duplicate, and send the form, as well as review recipients and responses, provided that they have the appropriate permissions to do so.
This feature comes with a new permission, called “Override Locked Forms”. This permission allows users to bypass the locked forms feature and edit, lock, and unlock locked forms. A user must have the “Edit Custom Forms” permission in order for them to use the new override permission.

The Manage Forms page remains mostly unchanged, but has a new “Select All” button on the field selector. This allows users to select all fields of a field group and add them to a page.

Manage Recipient Groups from within a form:
Finding and managing recipients is now more tightly linked to the form creation window. Instead of leaving the form to find recipients, you can now click “Manage Recipients” on the Form Builder to go directly to the new Manage Recipient Groups page.
The New Manage Recipient Groups page allows users to manage recipient groups for a single form in one place. To create a new Recipient Group, users can click the “Create Recipient Group” to create a new window and find recipients.
The actions menu to the right of each Recipient Group allows users to edit the window, find and remove recipients, as well as review responses.
What is a Recipient Group?
A Recipient Group is a combination of a form and a window start and end date. If you are familiar with our existing “Manage Custom Form Recipient Groups” page, each row that you see on that page is a recipient group. With the new enhancements to Custom Forms, we’ve simply given a name to each of these form windows.

Existing Manage Custom Form Recipients Workflow
The existing Manage Custom Forms page (Cogwheel > Manage Custom Form Recipients) is still available for users. This page continues to show recipient groups for all forms, however, all actions have been consolidated under the actions menu.

- Moved navigation options out of sub-nav bar and onto the page.

Sending a Form
Forms can be sent either through the new workflow (Edit Form > Manage Recipients > Create Recipient Group) or through the existing workflow (Cogwheel > Find Custom Form Recipients > Create Recipient Group). We’ve also added new "Select All" and "Select Page" buttons to the top of the “Find Recipients” page.

Managing Responses
Responses for a form can now be viewed directly for a single form, or can be viewed using the existing “Manage All Responses” or “Manage Custom Form Recipients” pages.
- Filters can now be hidden and unhidden using the new “Filter List” button.
- Added new filters for recipient group and school.
- Added new column for current school.
- Removed column for recipient group (this can now be accessed through the actions menu).
- Grouped actions into a single actions menu.
Other Enhancements

[Enhancement] Home Connection Portal: Redesigned Parent Email Notifications so they are easier to read and have a cleaner look.

[Enhancement] Pre-Registration: New locked form feature for pre-registration (this is similar to locked form for Custom Forms). There is a new permission for this feature,"Override Locked Pre-Registration Forms".
[Enhancement] Pre-Registration: Like Custom Forms, Pre-Registration Forms also can now be fully translated into Spanish using our "Manage Translations” page.
[Enhancement] Health: Redesigned the layout of the Health General widget to make it easier to read and find information.
[Enhancement] Health: When editing a procedure or medication, the user now sees a pop up box, instead of being redirected to another page to edit.
[Enhancement] Health: Added a success and a failure message when saving changes to the Health Cautions page.
