This article will walk through the Missing Medicaid Qualified Provider Widget
Tip:
Anyone who is a Medicaid Qualified Provider (such as a speech pathologist, physical therapist, occupational therapist, nurse, social worker, etc.) will see any student that they were either: 1) The Provider on the IEP, IFSP, etc. when the process was published OR 2) are now the provider
Filter/Column General Information:
1. The Report Option Filter allows you to see both Part B and IFSP (Show All) or select to ONLY see Part B or IFSP Report results within the widget.
2. The Students First Name Column and Last Name Column that are missing a completed MQP form names will show here and be able to be sorted using the arrow in the top right hand corner of either the First Name or Last Name columns.
3. The Process Name Column reflects the process that had YOUR SERVICE within the IEP, IFSP, P2P, IEP Amendment, NPSP, or NPSP Amendment at the time IT WAS PUBLISHED.
4. The Published Date Column reflects the date the process was published.
5. Your name will show in the Provider Name(s) Column IF you were either: a) The Provider who was listed within the Process (IEP, IEP Amendment, IFSP, P2P, NPSP or NPSP Amendment) when it was published WHERE A Medicaid School Service was included in the process OR b) transferred to the student AFTER the process was published WHERE A Medicaid School Service was included in the process.
NOTE: This means that you might be listed along with another provider of the same service type.
Clean UP: You will want to clean up the widget by having them complete a Medicaid Qualified Provider Form for any process that was published where a form was not completed. This will remove the student from the widget.
MOVING FORWARD: Use the widget to ensure that a Medicaid Qualified Provider Form is completed for EACH published process to ensure compliance with Medicaid Policy.
