This document will go over entering Immunization data on the Student Health Profile.
Where to Start
Go to Health > Student Health Profile
Under Health Immunization, click Edit
Enter dates for the appropriate vaccine(s).
When a student needs to show an exemption for a vaccine, you can use the Immunization Exemption widget.
For temporary and permanent, both will auto-populate the Health Card prebuilt report.
Selecting Medical Temporary will populate the temporary field on the Health Card and requires the Temporary End Date.
When Medical Permanent is selected for Immunization Exemption Reason Code field, dates are not required.
Health Immunization Requirement
To add additional immunization requirements, go to the Health Immunization Requirement widget.
Click Add if the student has no Health Immunization Requirement data entered, or click Edit Details when the student has data.
The Certified, Certified Date, and Certified Name (Staff Initials) fields will populate under the Status of Requirements area of the Health Card.
The All Requirements Are Met, All Requirements Are Met Date, Currently Up To Date, and Currently Up To Date Date fields will populate under the Status of Requirements area of the Health Card.
The All Requirements Are Met 7th, All Requirements Are Met Date 7th, All Requirements Are Met Name 7th (Staff Initials), Currently Up To Date 7th, and Currently Up To Date Date7th, and Currently Up To Date Name 7th fields will populate under the Status of Requirements area of the Health Card.
Downloading Health Card for Individual Students
On the Student Health Profile page, click Download Immunization Record
When downloading this for an individual student, data entered from the various health widgets will populate the CDHP 286.
This PDF can be edited to adjust fields as desired.
Editing fields on the PDF when downloading the health card will not sync to the Student Health Profile. Data is static.
