The Consent/Comments Page within the Plan of Care is to document the Qualified Staff's acknowledgement that, All services are provided with the expectation that the students primary care provider (PCP) and if applicable, the students case manager is informed on a regular basis. Additionally, the Qualified Staff will also sign and date the PDF within this section.
1. Check the box if you understand and agree, as the Qualified Staff, that, All services are provided with the expectation that the students primary care provider (PCP) and if applicable, the students case manager is informed on a regular basis.
2. Type the Qualified Staff Provider Name.
3. Enter any Additional Needs/Comments.
4. Enter Qualified Provider Credentials.
5. Click Save.
Enter Electronic Signature (Optional unless you have the permission turned on)
1. Sign your name.
2. Select OK after previewing your name.
3. Select OK to save your signature. NOTE: Once you select OK you can no longer edit your signature.
