The Parent Consent to Treat will print with the Plan of Care PDF if:
1. There is not a Current Consent to Treat Date on the C4S Key Data Page.
2. The Current Parent Consent to Treat Date is older than one year on the C4S Key Data Page.
Option One:
Go to Special Ed->Process->Blank Form Management
Option Two:
Open Quick Search->Type Blank Form Management-> Select from Dropdown.
1. Select Consent to Treat From Dropdown.
2. Select Download
1. Open Quick Search by either: a) Clicking Search OR b) CMD/K (PC users) CTRL/K (Mac Users)
2. Begin typing Consent to Treat
3. Select Consent to Treat from Dropdown.
1. Select Special Ed
2. Select Parent Consent to Treat (under C4S Heading)
1. Select Create New Parent Consent to Treat Details.
1. Select the District from the Dropdown.
2. Select the Date you are creating the Consent to Treat. NOTE: This date does not go on the Parent Consent to Treat it is just for compliance tracking purposes.
3. Select Save.
